Your period comes when it feels like it. Weight is creeping up even though nothing about your eating has changed. Acne along the jawline that never fully clears. Hair thinning at the parting — and, oddly, more hair on the chin.

If several of these are true at once, PCOD is worth asking about. It is one of the most common hormonal conditions in Indian women, and one of the most commonly missed — because each symptom on its own looks like something else.

Because ovulation doesn't happen regularly, the hormonal sequence that produces a normal period never completes. Cycles stretch out, skip, or become unpredictable.

Alongside this, most women with PCOD have some degree of insulin resistance — their cells respond poorly to insulin, so the body makes more of it. High insulin nudges the ovaries to produce more androgens (male-type hormones), which drives the acne, the facial hair, and the scalp hair thinning. It also makes weight loss genuinely harder — this is biology, not a lack of willpower.

PCOD vs PCOS — does the difference matter? In Indian clinical conversation the two terms are used almost interchangeably. Technically PCOS is the fuller syndrome with metabolic involvement; PCOD is often used for the ovarian picture alone. For a patient deciding what to do next, the practical management is the same.

PCOD Ke Lakshan — The Symptoms to Recognise

1. Irregular periods (the signature sign)

Cycles longer than 35 days, fewer than 8 periods a year, or gaps of 2–3 months. Some women bleed very heavily when the period finally arrives, because the lining has built up for months.

2. Weight gain, especially around the abdomen

Weight that goes on easily and comes off reluctantly, concentrated around the waist. Note that lean PCOD is real — a normal-weight woman can absolutely have PCOD, and it often gets missed for exactly that reason.

3. Acne that doesn't behave like teenage acne

Deep, tender spots along the jawline, chin and neck, often flaring before a period, and not responding well to ordinary face washes.

4. Unwanted facial and body hair (hirsutism)

On the upper lip, chin, chest or abdomen — coarse and dark rather than fine. Driven by raised androgens.

5. Hair fall from the scalp

Thinning at the crown and a widening parting, following a male-pattern distribution.

6. Dark, velvety patches of skin (acanthosis nigricans)

Around the neck, underarms, or groin. This is a visible marker of insulin resistance and is worth showing your doctor.

7. Difficulty conceiving

Because ovulation is irregular, conception can take longer. Important: PCOD does not mean you cannot get pregnant — many women with PCOD conceive, with or without treatment.

8. Mood, sleep and energy changes

Low mood, irritability, anxiety and fatigue are commonly reported. Some women also experience disturbed sleep. These are part of the picture, not a separate personal failing.

You do not need all eight. The pattern that should prompt a check-up is irregular cycles plus two or more of the others.

The Ayurvedic Perspective

Ayurveda does not have a single term that maps onto PCOD, but the presentation is classically understood through Artava Dushti (vitiation of the reproductive tissue) and Nashtartava (loss or obstruction of menstruation), with contributions from Granthi (glandular/cystic formation).

The reasoning: weakened Agni (digestive-metabolic fire) leads to Ama accumulation. Excess Kapha blocks the channels (artavavaha srotas), and disturbed Vata — specifically Apana Vata, which governs downward flow — disrupts the timing and rhythm of the cycle. The visible result is stalled ovulation, heaviness, and irregular menstruation.

Because the root is understood as metabolic rather than purely gynaecological, classical management focuses on kindling Agni, clearing Kapha and Ama, and restoring Vata's natural rhythm.

Herbs traditionally described in this context:

Shatavari (Asparagus racemosus) — the principal rasayana for women's reproductive health in Ayurveda, traditionally used to support hormonal balance and reproductive wellbeing. (Available as Shatavari Capsules.)

What Actually Moves the Needle: Diet and Lifestyle

This is the part with the strongest evidence behind it. Studies consistently show that a 5–10% reduction in body weight can restore ovulation in many women with PCOD — often more effectively than medication alone.

1. Fix the carbohydrate pattern, not the quantity. You don't have to give up rice or roti. Reduce the refined carbs — maida, white bread, biscuits, sugary chai, sweets, cold drinks. Choose jowar, bajra, ragi, brown rice, oats. The goal is a slower glucose rise.

2. Protein at every meal. Dal, rajma, chana, paneer, curd, eggs, sprouts. Protein blunts the insulin spike and steadies appetite. Most Indian vegetarian breakfasts are carbohydrate-heavy — this is the easiest meal to improve.

3. Move for insulin, not for the scale. Brisk walking 30–45 minutes most days, plus strength training twice a week. Muscle is the tissue that clears glucose. Strength work matters more than cardio here, and yoga (particularly Surya Namaskar, Bhujangasana, Baddha Konasana) supports both stress and pelvic circulation.

4. Take sleep seriously. Under 7 hours worsens insulin resistance measurably. Consistent sleep and wake times matter more than total hours alone.

5. Manage stress deliberately. Chronic stress raises cortisol, which worsens insulin resistance and disrupts cycles. Ten minutes of pranayama or meditation daily is a real intervention, not a nice-to-have.

6. Cut ultra-processed food. Packaged snacks, instant noodles, bakery items, cold drinks. These do more damage in PCOD than home-cooked ghee ever will.

7. Track your cycle. Any simple app or diary. Patterns you can see are patterns you can discuss with your doctor.

When to See a Doctor

Consult a gynaecologist if:

  • Your periods are consistently irregular, or you've missed 3 or more in a row
  • You are noticing increasing facial hair, persistent acne, or scalp hair loss
  • You have been trying to conceive for over 12 months (or 6 months if you are over 35)
  • You have dark velvety skin patches, or a family history of diabetes
  • Bleeding is unusually heavy, prolonged, or between periods
