It's one of the most common fears for men over 45, and it deserves a clear, calm answer. This guide explains how benign prostate enlargement (BPH) and prostate cancer actually differ — reviewed by an Ayurvedic doctor — so you know what's likely, what's not, and when to get checked.

First, What Is BPH?

Unlike BPH — which grows in the central part of the prostate near the urethra and so tends to disturb urination early — many prostate cancers begin in the outer part of the gland. That's why early prostate cancer frequently causes no symptoms at all, and why screening conversations matter: it can be present before you feel anything.

The reassuring part: when found early, prostate cancer is often very manageable, and many cases grow slowly. But "found early" is the key phrase — and that depends on getting checked rather than waiting for symptoms.

  • A weak or slow urine stream
  • Difficulty starting, or dribbling at the end
  • Needing to go more often, especially at night (nocturia)
  • A sudden, urgent need to urinate
  • A feeling the bladder isn't fully empty

In BPH these come from the enlarged gland squeezing the urethra. In cancer, similar symptoms — if they occur — usually appear later, once a tumour is large enough to affect the flow.

Symptoms to take more seriously and see a doctor about promptly:

  • Blood in the urine or semen
  • Bone pain, especially in the hips, back, or thighs
  • Unexplained weight loss or persistent fatigue
  • New erectile difficulties alongside urinary changes

These aren't proof of anything on their own, but they warrant timely medical assessment rather than home remedies.

The PSA Test: Helpful, but Not a Verdict

PSA (Prostate-Specific Antigen) is a protein measured by a simple blood test. A higher level can be a flag — but it's not a yes/no cancer test.

PSA can be raised by BPH, a urinary infection, recent vigorous exercise or cycling, ejaculation shortly before the test, or a recent catheter or exam — not just cancer. Equally, some prostate cancers occur with a normal PSA.

  • Manage stress and sleep, since both affect Apana Vata balance and urinary patterns.
  • Get screened on schedule — the most protective habit of all.
